Medical, Dental, Vision and FSA Benefits
Promoting a Healthy Life
Medical Coverage: LS Technologies offers four CIGNA national Preferred Provider Organization (PPO) plans for our employees located all over the country. Employees choose among a high deductible Health Savings Account (HSA), medium deductible Health Reimbursement Account (HRA), low deductible, and no deductible in-network only options according to preference. The plans cover a wide range of medical services including 100% coverage of routine annual physicals and related tests if performed by an in-network doctor. There are no pre-existing condition exclusions and no overall lifetime benefit maximums.
Prescription Benefits: Employees enrolled in any of the four medical plans receive prescription benefits through CVS/Caremark. Mail Order options are available through the plan, as well as a Performance Drug List to help save on prescription costs.
Health Savings Account (HSA): Employees selecting the High Deductible PPO plan receive a Health Savings Account, and employer contributions of 50% of deductibles to use on medical-related costs.
Health Reimbursement Account (HRA): Employees selecting the Medium Deductible PPO receive a Health Reimbursement Account where the employer covers the remaining 60% of the deductible after the first 40% is met by the employee.
Flexible Spending Account (FSA): Employees selecting this benefit receive an account to contribute pre-tax dollars to use for qualifying healthcare (HSA subscribers ineligible), dependent care, and transportation expenses (parking and transit).
Dental Coverage: Two dental options through the MetLife national dental PPO network. The plans cover preventive check-ups, basic dental treatments, and restorative procedures. Orthodontics also covered with the upgrade plan. Two annual preventive care visits per person are covered at 100%.
Vision: Coverage through VSP’s national provider network includes routine eye exams for a small copay, and yearly allowances for prescription eyewear at already affordable prices and contacts.
Insurance & Disability
Protecting you during uncertain times
Employee life insurance: $50,000 worth of coverage; includes additional coverage for accidental death & dismemberment.
Short-term disability coverage: Coverage of 60% of your income if you are disabled and cannot work for more than seven days.
Voluntary Long-term disability coverage: Coverage of 60% of your income if you are disabled and cannot work for more than 90 days.
Supplemental Employee life insurance: Coverage from one to five times your annual salary for employee. Spousal and child coverage is also available, as is accidental death & dismemberment.
Supplemental Accident and Critical Illness coverage: Employees may purchase additional financial protection in case of severe injury or illness.

401(k): Employees are eligible to contribute to Mass Mutual 401(k) accounts through payroll deduction. The company will make a safe harbor employer matching contribution of up to four percent of your salary – calculated as 100% of the first three percent of pay you contribute plus 50% of the next two percent you contribute. Safe harbor matches are immediately 100% vested.

Annual Leave: We offer a competitive program to give you time away for your personal priorities. Annual leave encompasses traditional vacation time along with paid personal time off and sick leave, providing flexibility for how, when and for what purpose time off is taken. The number annual leave days is based on length of service, but ranges from 15 to 20 days per year.
Holidays: LS Technologies provides 10 paid federal holidays.
